Vancomycine opladen met 20mg/kg. Daarna start continue infusie zoals hiernaast weergegeven. Daarna op geleide van continu spiegel. Rifampicine pas starten bij aangetoonde gevoeligheid. Controleer regelmatig leverenzymen tijdens therapie met rifampicine. |
